Billing Information

We would like to take this opportunity to explain our office policies to you.

We are committed to providing you with the best possible care. If you have medical insurance, we will try to help you receive your maximum allowable benefits. In order to achieve these goals, we need your assistance and your understanding of our payment policy.

A 24-hour notice is required when canceling an appointment. A missed appointment, without notice to our office, can result in a charge to you. This is a service charge and will not be billed to your insurance carrier.

Payment for services, including office visit co-pays, is due at the time of service. We accept cash, check or credit card. We will bill your insurance company; however, you are ultimately responsible for payment.

A parent or legal guardian must accompany a minor (child) at each office visit. The person who accompanies the child to the office is responsible for payment. When another party is legally responsible we ask that you seek payment from them.

Insurance

Capital Urological Associates is a provider for Medicare and most major insurance plans, and provides insurance billing. You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please contact us. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.

Your insurance is a contract between you, your employer and your insurance company. We are not a party to that contract. Not all services are covered benefits in all contracts and some insurance companies arbitrarily select certain services that will not be covered. You will be responsible for services that your insurance carrier does not pay for.

An itemized receipt or encounter form can be provided for you to submit to your insurance carrier for office visits and consultations. All billing questions should be directed to our billing department at (517) 349-3900. Our billing office hours are Monday through Friday 8:00 a.m. to 5:00 p.m.

Please have all insurance cards available the first time you come in. If your insurance card(s) is(are) not available, your appointment may be rescheduled.

We ask that you keep us advised of any changes in address, phone number or health care coverage or any other pertinent information.

Dr. Stockall participates with the following insurance providers: Medicare, PHP, Blue Care Network, BCBS Traditional and PPO, SPHN, Aetna, CIGNA, United Healthcare, Medicaid, PPOM, among others. Some insurance plans require authorizations/referrals from your primary care physician to see a specialist or to receive certain services. It is your responsibility to obtain authorization, if your plan requires it.

You as a patient are ultimately responsible for the charges for all professional services rendered by Capital Urological Associate Providers.
